According to a New York City Board of Correction report, Javier Velasco died on March 19, 2021 while held at a jail in New York City, after hanging himself shortly after he was removed from suicide watch. The report found that officers waited four minutes before rendering aid. It also found that medical staff and EMS were not called for nine minutes.
