A young father died at the Tafuna Correctional Facility in American Samoa, and the warden said he died by suicide. The Samoa News article notes that his wife was the family member it mentions, and that he already knew what had happened to his family. He was placed in room 18 because other rooms were overcrowded, and he was found early the next morning. At a House hearing, Warden Papalii Marion Fitisemanu admitted that staff had lapsed and made mistakes, and that the facility was at fault. Police are still investigating the events of that morning.
