A person held at Bilibid Prison in Manila died in November 1906 after receiving an experimental anti-cholera inoculation at the prison, one of about ten to thirteen deaths reported from the same inoculations. The Secretary of War's annual report, citing Governor Smith's cablegram, said the cause was still under investigation and that the serum may have been mixed with plague culture by mistake. A committee of scientists later found the deaths came from attenuated plague cultures mixed into the cholera serum. A committee of Filipinos found that no one was responsible for the accident and recommended provision for the families of the dead.
