A person died at the Warren County Jail in McMinnville, Tennessee, on January 14, 2019. The Tennessee Bureau of Investigation and district attorney's investigators looked into the death, and their investigation found that two corrections officers did not complete the required jail walk-through that day. The investigation also found that jail logs were altered to show the walk-through had been done. In September 2019 a grand jury indicted both former officers on tampering with governmental records charges.
