A 45-year-old man died by suicide by hanging at the Tecumseh State Correctional Institution in Nebraska in 2022, about two weeks after he cut himself and was moved back to his regular unit from suicide watch. The Nebraska Inspector General, Doug Koebernick, reported in a review of the Nebraska Department of Correctional Services that critical incident reviews and phone-call checks in the case were not completed.
