Patrick MacDonald, 33, of Columbus, died on September 28, 2020, at the Delaware County Jail in Ohio, where he had been held for about four days. Corrections officers found him hanging in his cell on the Monday night, and he was pronounced dead at Grady Memorial Hospital. The sheriff's office said an autopsy would be performed and that no foul play was suspected, and Chief Deputy Jeffrey Balzer called the death a tragedy for the young man, his family and friends. According to The Columbus Dispatch, MacDonald had passed the jail's self-harm screening questions, and the jail later sought a stronger grate design after his death was found to be similar to an earlier suicide there.

Cause of death, as reported: Died by suicide (neglect alleged). The Delaware County Sheriff's Office said Patrick D. MacDonald was found hanging in his cell, and a previous suicide at the jail was reportedly done the same way, which prompted the jail to seek a stronger grate design; one account says he had passed the jail's self-harm screening questions.

Reported by: Delaware County Sheriff's Office (the agency that held him). The only account of how this person died comes from the agency that held him.
