Joshua Daczewitz
Joshua Daczewitz, 22, was held at Menard Correctional Center in Chester, Illinois, when he was killed in 2004 by his cellmate in a segregation wing. According to the NPR account, the cellmate had repeatedly asked guards to move him, and the cellmate wrote a note to guards and later pleaded guilty to the killing. Daczewitz's mother later won a lawsuit over the placement and a separate settlement with the state. The account says he grew up in a suburb of Chicago, and his mother pursued the case after his death.
Cause of death, as reported: Killed by another person in custody (neglect alleged). The cellmate, Corey Fox, wrote a note to guards saying he would kill Daczewitz if they did not move him, and later pleaded guilty to the killing; Daczewitz's family's lawsuit says staff failed to act on the warning.
Reported by: Cellmate Corey Fox (witnesses); Daczewitz's mother's lawsuit (family or their lawyers).
Lawsuit: His mother sued over the placement and settled with the state, but the source gives no dollar amount for the settlement.
