Jose Hernandez
Jose Hernandez, 67, had dementia, and his family wanted him to go home to a relative who could care for him full time. His medical parole request was denied in October 2024 and approved in June 2025 after his lawyer filed reconsideration requests, but the state Department of Correction required a skilled nursing home instead. He was held at a Holyoke facility before being moved to Baystate Medical Center, where he died in November 2025. His death certificate lists organ failure from liver disease as the cause, and his daughter, Ana Maria Hernandez, stayed at the hospital so he would not be alone.
