Fredreaka Jack
Fredreaka Jack, 37, was a gifted child and once a vibrant dancer and pianist, according to her family, who say she earned her GED and attended Loyola University New Orleans. She had schizophrenia and bipolar disorder and was discharged from Patton State Hospital in 2022 to serve parole at Walden House, a residential program in El Monte, California. She died in July 2022, about 14 hours after an emergency room visit, and the autopsy cited complications from Type 2 diabetes. Records show gaps in her medication and missed signs of diabetes before her death, and her mother says she was robbed of the chance to bring her daughter home, while the state, the contractors and the clinics disputed responsibility or did not respond to questions about her care.
