According to Krish Gundu, executive director of the Texas Jail Project, a man in acute psychiatric crisis was held in a single padded cell at the Harris County Jail in Houston, where he was restrained. Gundu said the man died by repeatedly striking his head on a metal floor grate, which he described as a suicide, while jail staff watched. Gundu said the man should have been sent to a hospital instead of being held in that cell.
