Jack McKenzie

20 years old · died September 3, 2021 · Polmont Young Offenders Institution

His family described Jack McKenzie, 20, as a son, nephew and brother who was loved by them. He died in his cell at Polmont Young Offenders Institution near Falkirk, Scotland, on 3 September 2021, and a sheriff's determination at a fatal accident inquiry concluded that he died by suicide. The sheriff found that his death could have been avoided if the Scottish Prison Service had replaced a cell door known to present a suicide risk, and that night-time hatch checks were failed. The Scottish Prison Service and the Scottish government have apologised to his family.

Cause of death, as reported: Died by suicide (neglect alleged). Sheriff Simon Collins found in a fatal accident inquiry that Jack died by suicide and that his death could have been avoided, citing Scottish Prison Service failures to replace a cell door known to present a suicide risk and failures in night-time hatch checks.

Reported by: Sheriff Simon Collins, fatal accident inquiry (court).
